Do I need a D4 medical for my HGV licence?
A valid D4 medical report is vital for maintaining your heavy goods vehicle driving licence (or any other group 2 vocational driving licence).
- You’ll need to get a D4 medical when you first apply for a provisional D2 licence to drive HGVs. This D4 remains valid after you’ve passed your Driver CPC qualification.
- Your licence will need to be renewed every five years, but you don’t need a new D4 for each licence renewal until you’re 45 or older.
- From age 45, you’ll need a new D4 medical every five years to maintain your licence.
- From age 65 onwards, you’ll need to renew your licence every year. A new D4 medical is required every year to secure your renewal.

Who can carry out a D4 HGV medical?
D4 medical examinations and reports for HGV drivers must be carried out by a UK licensed and General Medical Council (GMC) registered doctor.
At DVLA Driver Medicals, all our doctors are fully UK licenced and registered with the GMC for total peace of mind.
What’s involved in a D4 medical?
Most D4 medical examinations for LGV and HGV drivers include the following:
- Medical history questionnaire: covering a comprehensive range of medical factors including epilepsy, diabetes, hypertension and more
- Cardiovascular exam: covering the heart, lungs and blood vessels
- Blood pressure test: checking if your blood pressure is too high/low or at a healthy level.
- Eyesight test: identifying if your vision is sufficient to drive a HGV or needs correction
Your employer may also require you to complete a urine sample test, which we can provide as part of your D4 medical.
How much does a HGV medical cost?
Most D4 medical providers will charge between £150 and £180 for a single medical exam; but not us!

Where will I need to go for my test?
It’s your choice!
Our team can arrange a convenient venue local to you within the Southampton/Portsmouth area; or alternatively we can carry out your medical exam on-site at your employer’s premises.
What will I need to bring to my medical exam?
You’ll be asked to bring:
- A valid photo ID (such as a driving licence or passport)
- Your medical history record
You should also bring the following if they’re applicable to you:
- Your glasses and/or contact lenses and optical prescription
- Your medication prescriptions
If you’re unable to bring these items, please let us know as soon as possible.
Help! I’ve forgotten or lost these items on the day of the test; what should I do?
Please remember to bring these items with you, but don’t panic if you’ve forgotten them or can’t find them; just let us know as soon as possible.
Photo ID is required as an absolute minimum in order to process your exam. If you’re unable to bring this, we may be able to reschedule your airside medical appointment at no extra cost depending on notice given. (Please note that an appointment may only be rescheduled once, and appointments are not transferable between individuals.)
I’m worried about the exam; will that affect my blood pressure reading?
Stress and anxiety can temporarily raise our blood pressure, which can affect blood pressure check readings.
Rest assured; unlike other D4 medical providers, our medical examiners will give you plenty of time to settle in. If your first reading is particularly high, we’ll give you a 15 minute interval to relax before taking additional readings.
What happens if I fail my HGV medical?
Don’t worry; if you happen to fail the first time from a correctable issue (such as a need for glasses to correct vision), we’ll usually offer a free return consultation.
What if I need to cancel my test appointment?
Please get in touch with us as soon as possible if you need to cancel your appointment. Depending on the amount of notice given, we may be able to reschedule your appointment at no extra cost to you.
