Do I need a D4 medical for my taxi licence?
Local councils across the south – and indeed the rest of the UK – generally require taxi drivers to hold a valid medical report in order to obtain and renew their licence.
The exact medical requirements for taxi drivers will differ from one council to the next, but most will require a medical report to D4 standards; the typical standard for most commercial vehicles in the UK.
When will you need to renew your taxi medical? Again, the requirements will differ depending on the area you’re operating in.
Some local councils will require that you get a new medical every five years once you reach the age of 45, as with most D4 medicals; while some may require you to renew it every three years or sometimes more frequently.

What do I need to bring to my taxi driver medical?
The most important thing you’ll need to bring is a completed council taxi medical form. You can get a copy of the form from your local council by downloading it from their website or contacting them directly.
- Southampton City Council taxi medical form
- Portsmouth City Council taxi medical form
- Winchester City Council taxi medical form
Please note that different local councils have different criteria for assessing taxi licence applications.
The City Council of Wolverhampton offers a simplified medical form which enables private hire vehicle and taxi drivers to work anywhere in the UK; as long as they’re licensed in Wolverhampton and working for a Wolverhampton-licensed operator.

You’ll also be asked to bring:
- A valid photo ID (such as a driving licence or passport)
- Your medical summary from the GP (usually known as a Subject Access Request)
- Your glasses and/or contact lenses and optical prescription (if applicable)
- Your medication prescriptions (if applicable)

What’s involved in a taxi driver medical check?
Different councils will have different medical standard requirements, but many medical tests fo taxi drivers will include:
- Medical history questionnaire: covering a comprehensive range of medical factors including epilepsy, diabetes, hypertension and more
- Cardiovascular exam: covering the heart, lungs and blood vessels
- Blood pressure test: checking if your blood pressure is too high/low or at a healthy level.
- Eyesight test: identifying if your vision is sufficient to drive a taxi or needs correction
Employers may also ask you to complete a urine sample test; which can be provided by our doctors as part of your taxi medical.

Don’t worry; while stress and anxiety can temporarily raise our blood pressure, our doctors will give you plenty of time to relax and settle in. Plus, if your first blood pressure reading is too high, you’re free to take a 15 minute break before taking additional readings.
What if I need to cancel my test appointment?
Please get in touch with us as soon as possible if you need to cancel your appointment. Depending on the amount of notice given, we may be able to reschedule your appointment at no extra cost to you.
